Acute respiratory infections (ARI) are a leading cause of morbidity and mortality in children under five, especially in developing countries. Mothers’ awareness of ARI symptoms is essential for determining appropriate attitudes and actions in managing ARI in children. Lack of maternal knowledge may increase the risk of ARI in children. This study used a descriptive observational design with a cross-sectional approach. The population consisted of 40 children aged 1–5 years, with 30 respondents selected using incidental sampling. Data were collected through questionnaires, interviews, and observations, and analyzed using univariate and bivariate methods with the Spearman rank test. Most respondents (60%) had low knowledge, and the majority of children experienced moderate ARI (60%). The Spearman test showed a p-value of 0.000 (<0.05) and a correlation coefficient of 0.889, indicating a strong relationship between maternal knowledge level and ARI incidence. These findings suggest that low maternal knowledge contributes to the high incidence of ARI among children aged 1–5 years. There is a significant relationship between maternal knowledge level and the incidence of ARI in children aged 1–5 years. Increasing maternal knowledge through health education is essential to reduce the incidence of ARI in children.
